File content should be escaped #1

The files content isn't escaped. So if there's HTML in it (like in dynamic_form for example), the html fields are rendered in the page unescaped.

Thanks, I didn't think of that yet!

This is an issue with the html formatter though, so I recreated the issue at simplecov-ruby/simplecov-html#2
