Identifying Run As Package on missing Environmental Variable #299

When using the 0.94 version of the extension, the application is erroneously identifying the web app as being run from package when the WEBSITE_RUN_FROM_PACKAGE key is not present. We were able to work around this issues by placing this key in the app settings and setting the value to 0.
