Bugfix release that also activates the plugin on the new Toggl web app URL: https://toggl.com/app instead of https://www.toggl.com/app.

Almost the same functionality as before, but:

• Manual page refreshes are not needed anymore (the timer refreshes itself when needed).
• The progress of the timer is saved even when the tab is closed and reopened later

First minimal version

A first minimally functioning version. Still has some issues, but reloading the page should resolve most of them.
