PHP Upload Sample app

Steps to have this code working:

1. Clone the repository
2. Create the table "images" using the DDL included on the folder "db"
3. Change the configuration on the file configuration.ini on the folder "src"
4. Copy the files on "src" folder to the root of your webserver
