v4 migration docs don't mention that badges in .list-group are no longer automatically pulled right

The utility classes have changed in v4—.pull-xs-right is what you need. I'm still debating that class change but for now it's what we're using.

@mdo it isn't the problem that the class has changed, it can indeed be appended. The problem is that all examples mention that badges and now labels are right aligned. This is not the case anymore.

Docs were updated to not mentioned this at some point. Making mention in the migration docs, but that's about all I see that needs doing here. See 7fa88b3.
