Fixed a bug when adding chapters to book

New trigger

Changed the question trigger to {{? because this is more similar to shortcode {{< and slightly faster to type

Also for books with parts

Fixes:

• new engine makes it possible to produce reasonable counts for books also in preview mode.
• now books with parts can be handled, too.

now for books (quick and dirty fix)

Questions are numbered throughout the book.

This gives funny results when previewing a html book, because currently the start number for every chapter is read from a file that is updated after rendering each chapter. It works well when rendering an html book at once.
