Initial code cleanup

Fixes some markup validation issues (caused by mismatched opening/closing
tags), harmonize use of `<div class="note">` vs `<section class="note">`,
remove some redundant block level elements, add `<p>...</p>`
where needed, initial attempt at fixing main code indentation
