this fixes #127, #129, #155.
however, we have some impedance mismatch between how we've been intending to generate attestation format IDs, and extenstion IDs. This is not (yet) resolved in the spec text in this set of commits in this branch (see below) -- we should discuss and resolve and then merge, ok?
