This happens when you hit the side of a block from the top I think and the character sinks in.

It is still there, it's just very rare

I have been doing more testing and this only occurs when jumping from higher velocities (approximately 5-6 tilesizes )

Hmm.. is there any chance that because it's moving so fast, you're referring to "adjacent blocks" that are no longer adjacent?

I added terminal velocity a while back so it never gets the wrong block..
