Consolidate show filter and show column screen options

In an effort to reduce the number of Screen Options, I think it makes sense to consolidate the Show filters area into the Show on screen checkboxes.

If you're going to hide a column, then you probably also want to hide its filter, and vice-versa.

Of course there could be cases where this is undesired, but IMO it's not worth the extra five options of clutter it adds to the UI, and it doesn't match our philosophy of less is more.
