In rare situation when both Layout Publish and Layout Status are executed at the same time, both of those transactions can trigger Layout build, which may lead to a Layout with publishedStatusId = 2 (draft) after publishing.
Rendering the Layout inaccessible via web ui - throwing Layout not found error when Layout Designer is open.
