Note: Re-issued this pull request because I couldn't resolve the problems with the mixed commits on the other one.
Added extractor for fetlife. You need a paid account for fetlife to view videos (a free one is not enough), and since I will not offer mine I am aware that this extractor will probably not be added. But with this pull request, others will be able to find my extractor and build their own youtube-dl with fetlife support.
