Summary:
Added "Custom SMTP Configuration" section to "Health & Alerting" tab at customer profile page.
Depends on D8528

Test Plan:
Go to customer profile, open "Health & Alerting" tab, enable "Custom SMTP Configuration" checkbox, enter SMTP configuration, hit "Save" button.
Open same profile page again and:
- make sure changes are saved
- make sure provided credentials are used
Go to customer profile, open "Health & Alerting" tab, disable "Custom SMTP Configuration" checkbox, hit "Save" button.
Open same profile page again and:
- make sure changes are saved
- make sure SMTP credentials are reverted back to default presets

Follow up to the API task, we need to add UI support for the user to specify the STMP credentials to use for alerts.
