The Pull Request Template isn't working

What I expected

Clicking the New Pull Request button would show the new template

What I saw

Just an empty description

Context

I think we just need to tweak the template file's location. The GitHub docs said it could go in the .github/PULL_REQUEST_TEMPLATE folder, but maybe it needs further customization to get that to work. Moving the file to .github/pull_request_template.md should work, according to the docs
