Any idea how this happens? I've also ran into this issue and re-downloading the blockchain is quite cumbersome, but seems to fix it. I'd like to know the root of the problem.

You can try re-index it - bitcoind -reindex, else you will have to wipe it
out and reload the entire blockchain. I ended up reloading the blockchain,
and it works 100% now.
