Add missing copyright notices and remove duplicate notices #2626

This pull request adds the copyright notice to files that, incorrectly, didn't have one.
Added, for some files it removes the duplicate notices.
This is a particularly bland pull request, although required.

Note that the finder of this misser is @timtebeek, who constructed pull request #2613 to resolve this.
Since the formatting was slightly off, I figured I'd construct the fix locally.
But, honor where honor's due: thanks for spotting this, @timtebeek!
