Adapted from https://github.com/Azure/azure-quickstart-templates/tree/master/201-vmss-ubuntu-web-ssl and no auto-rotation support (because it currently needs to be done after initial deployment). Created another issue for cert auto-rotation (#74) so that this PR can fix #14.
