Initial release - ready to be used on your own risk

Unpack it and copy the folder inside into ./extensions/hooks/ so that the included index.js is then located at ./extensions/hooks/version-control-changelog/index.js.
Make sure you add the required configuration items (see version-control-changelog) before you restart your Directus
