Add option to change PR title and description

Thanks for this very thoughtful action. It's perfectly suited to our needs!

You can already change the commit body, the commit/pr prefix and I have been thinking about using the message of original commit for sync commit (#13), so this is definitely a valid request as well!

It'd be nice if we could edit these fields via the sync.yml config or via params to the action.

We could add another action input like PR_TITLE and PR_DESCRIPTION similarly to the COMMIT_BODY option.

How would you imagine theses options to work? Would it overwrite the title and description completely or just change parts of it? For example the PR body contains all of the changed files so overwriting it completely would mean they wouldn't be included:

Okay, no problem! I still see the use case to at least add your own context to the PR, so a PR_BODY option might be a good addition.

Would you be willing to create a PR for this @dhruvkb? If not, totally okay, then I will work on this later this week.
