Describe the problem
Once a user is "Deleted" from an organization, their role is set to "orphaned," and their organization_id is set to null.
The next time the user logs in, the system creates all kinds of problems for them... we send them in circles.

Describe the solution
If a user logs in and their organization_id is null, then redirect them to the "Request Access" page, but add the following explanation: "Someone from your previous organization has removed you from that organization. If that was an error, or if you would like to join a new organization, enter the email address of someone you know from the organization you want to join."
