1.2.0: Add support for Phones

The original 1.1.0 ProxySwitch app was written for TP and didn't display well on the Pre 1/2 and Veer. Adjusted a few things so that it displays correctly on phones now as well.

Needed to rename the app in order to be able to submit it to PivotCE feed.
