All issues below are on the main OpenStreetMap.org domain, the router at map.project-osrm.org works as expected. Perhaps I'm just testing against an older version, and these are bugs that are already fixed?
On the main OSM site, the directions for this route give the text direction "take exit onto Lewis and Clark Highway (WA 14)", rather than the expected "take the second exit onto South 15th Street". Reversing the route gives the expected result.
the tagging of the roundabout way with a name/ref
-- Counterexample: these roundabouts with the same issue [1], [2] but the roundabout ways have no name/ref
Perhaps this is somehow related to the interaction of roads of widely different classes?
Observed issues:
trunk - unclassified
trunk - tertiary
primary - residential
Working as expected:
primary - secondary
tertiary - residential
I've not changed any of the tagging yet, since I'm not sure if this is a OSRM bug, and I wanted to leave it alone for people to look at.
I initially asked this question on the OSM Help forum, and was pointed here.

Similar issue at another nearby roundabout, for all three routes exiting to the South.
Suggestions in the thread include:
the tagging of the roundabout way with a name/ref
-- Counterexample: these roundabouts with the same issue [1], [2] but the roundabout ways have no name/ref
the (admittedly extraneous) relation forbidding a u-turn prior to the entrance, mirrored on the other side.
-- Counterexample: the second roundabout doesn't have these relations.
However, at another roundabout I recently mapped, all routes seem to work properly.

This may be related to issues #4716 and #2610.
