Event Calendar Item Personalized Registration block doesn't allow occurrence selection

Description

Using the Event Calendar Item Personalized Registration block, you are unable to select any of the event occurrences from the list.

Steps to Reproduce

1. Create a link to a page with the block on it.
2. Go to that page using a link both that passes both the EventItemId and PersonGuid and their values
3. The event occurrences are listed on the page
4. The check boxes aren't really check boxes and you are unable to select an occurrence

Expected behavior:

Be able to check a box and proceed to a registration page

Actual behavior:

Box doesn't get checked and if you click the start registration button you get an exception
