In this PR I remove some unused files, and change configure.ac so that parallel I/O is properly detected in versions of netcdf-c before 4.6.2. Also cause configure to fail if --enable-parallel-tests was used and parallel I/O is not available in underlying netcdf-c library.
This PR is ready to merge.
