tobaccopipes.com forms being filled in

Trying to fill name/address forms don't fill out completely.

For example on www.tobaccopipes.com when trying to check out, I will try to use my name form which in seprate sections has address 1, state, zip, name, etc.

In that particular website at least state and address 1 aren't being populated. If I try with LastPass, it works. I have imported my logins including the address forms from lastpass.
