This PR addresses #2337.
I renamed inputs and made the form handling based on an array config (
$uri_paramsand$server_params) so this form can be extended by config changes.Added a permalink to the form
We now support drawing from URL params (permalink).
Once approved I'll remove the leftover jQuery completely.
