onboard new members for ChainAgnostic

I wrote a message on the chain agnostic discord. Can we create a repo to onboard new members of the chainagnostic initiative?
Adli and Myself propose to help to get more blockchains member particularly to get their namespace for CAIP-2 and also we can structure a little bit the different working groups.
I did that as a draft that we could put in a chain agnostic repo:

Sorry missed the discord message. Had a short look - looks good and I think we should do it.
Just some nits:

• the repo name has a double alliance. So either just casa or chain_agnostic_standards_alliance
• please no mention/usage of telegram - we have the discord @pedrouid created we should use
• there is a reference to "Fill out this 30 second form." - where is this form?

Ok I renamed the repo and remove the reference to telegram and add a simple form to collect interest. Please check:

great - I created: https://github.com/ChainAgnostic/CASA so you can PR your stuff there.
Not a big fan of the typeform thing tbh:
a) we should not ask for government aliases
b) I would like a more open approach like doing it via a PR

maybe some others can chime in @pedrouid @webmaster128 @rekmarks @soc1c

I can remove the form and ask only the new member to do PR to add their contact that's totally cool!

@ligi I cannot fork because the repo is empty. Can you add a Readme?
