Note: only version 3.x if TinyMCE is supported.

Install TinyMCE's jQuery plugin

Install the plugin by copying the content of cloudinaryimage to your tinymce plugin directory

Add cloudinaryimage to your plugins list in TinyMCE initialization, e.g.,
tinyMCE.init({ theme : "advanced", plugins : "cloudinaryimage" });

To initialize the plugin you will need to add a server side endpoint to build a signed URL. For example in Ruby on Rails:

Use the signed URL built on the server to initialize the tinyMCE editor (textarea is the jquery element you wish to decorate), e.g.,
