Output format updates

Significant changes to output formats: cleaned up and enhanced HTML output, cleaned up PDF output, option for MISP output. Improvements to Output section of the User Guide.

Base release

Provides basic functionality of CDAS: threat actor, country, and incident simulation. This release also allows the user to easily provide custom files to be used by the simulator.
