Add all namespaces on creation of the text document

The XML namespaces for xmlns:text (paragraph), xmlns:xlink (hyperlink), xmlns:draw (image), xmlns:style and xmlns:fo (style) are added to the document as soon as one of the respective elements is added to the document.
This will produce small and clean documents but requires additional code and logic, that will get harder to test and maintain. I think it's better to add the known namespaces to the document directly.
