Shipping Zones and Tax Zones don't remember selected countries/states when there's a validation error

Steps to reproduce:

1. Create a new Tax Zone
2. Select some countries, but leave the Name field blank
3. Save

The page will reload with a validation error on the Name field as expected, however the Countries field will be blank rather than filled with the previously-selected countries.

Also happens if you had selected for it to be State-based instead.

Also happens when creating/editing a Shipping Zone.
