Author's notes are modelled as <blockquote class="afterward"> blocks. Do something nice with them.

Add an option "-a" to select style for author's notes

First stab at an infrastructure to handle #5.

Allow author's notes to be rendered as plain text (as before) ot not
at all (the new default), depending on the argument to the
"-a/--authors-notes" option.
