Document which output formats utility classes affect

Our supported classes include utility classes like .fixed and .image-height-*. It's not clear from the docs which output formats these classes affect.

For instance, .fixed and .image-height-* only apply to PDF outputs. If they will only ever do this, then we must note that in the docs, perhaps in a new column in the classes table.
