On a server running 4336f56:

I think this may be associated with always-on users with the +i mode? Maybe when they add and remove a single session?

This affects ordinary, anonymous users when +i is a default usermode. It seems to be a bad interaction between #1079 and #1303. This code is impossible to get right :-(
