Add complete support for CMap files

Before this change only the special ToUnicode CMap files were supported.

* Implement support for the missing 'usecmap', 'begincodespacerange',
  'begincidchar' and 'begincidrange' operators in the CMap parser.
* Implement the needed methods for dealing with the CIDs in the main
  CMap class.
* Add the predefined CMap files that are used by PDF and add support for
  parsing them by using their name.
