Remove CMakeLists.txt prototab.[ch] generation and perl discovery. Update .gitignore to not ignore prototab.[ch]
Remove the "AUTOGENERATED" warnings.

Check in static prototab.[ch] source files and remove mkprototab.pl. Add copywrite headers and tidy them with 'make tidyc'. Remove references to old autogeneration in .gitignore and CMakeLists.txt.
