Updating official/ to the latest changes.

Report on the official diff checking:

The only content changes were some word definitions that changed
upstream.

There are several places where the PDFs showed visual diffs (and a bunch
more than I already fixed):

page 290 -- changes to hyphenation rules
page 293 and 297 -- the previous version had no medium length dashes
                    where it should have had

There are various mismatches in the glossaries and index (i.e. from 505
on), but I've checked all the changes to the actual content and there's
nothing scary there.  A few word definitions changed slightly, but most
of it is just visual misalignment that I can't be bothered to figure out
how to fix.
