Clean up controller preset info

Patch does the following:
* Uses uniform naming scheme <vendor name><device name> in presets drop down menu
* Removes session id's from forum links
* Adds forum/wiki links if possible

One little annoyance: If a preset provide both a link to forums and wiki, it appears to be one link in the preset info (e.g. Quneo). Would be nice if we'd add a line break or at least a space between both.
