Currently, controllers have to be connected at the start to be recognized. This can be very impractical if I want to start playing something but am still setting up my gear. Is it possible to implement a refresh or something to reload them while mixxx is running?

This would be great, but I don't think it would be so easy to implement. The old PortMIDI library we use does not support hotplug and it has been unmaintained for years. We may consider switching to RtMidi for this.
