Environment

  • Python version: 3.7.13
  • Nautobot version: 1.4.0

Steps to Reproduce

  1. Login to Nautobot
  2. Go to any form that uses object_create.html template (e.g. dcim/device-roles/add/, ipam/vlan-groups/add/, circuits/circuit-types/add/)
  3. See that Note shows up twice in the form.

Expected Behavior

Notes shouldn't show up in the 1st block of the form but instead be in it's own block
