Moves where the user enters external insulin from the bolus sheet, to the treatment log.
This was based on Artificial-Pancreas/iAPS#271, as part of #47.
Follow-up PRs will add the rest of the features for #47, and add text to display SMBs, as well as filter out future entries.
This PR builds on top of #80.
