Speed up story ID lookups

Reduces the time to lookup a story ID by caching the Pivotal Tracker initials of each git author (cutting down on API calls).

Skip commit messages with a story ID

Inflight now skips if a story ID is already present (for example during a rebase or amend of an existing commit).
