Fixes for issue #108 and issue #107

* the xsl-ns build incorrectly added a namespace to URL parts in
  entities (e.g. .../d:release/xsl-ns/d:current/d:xhtml">. (#108)
* daps2013 and opensuse2013 had included files from the suse2013
  directory via relative path (../../2013); therefore the *-ns variants
  included the DocBook4 versions of these files
  - replaced relative paths with catalog-resolvable URNs
  - added local catalog entries to etc/catalog.xml, so the stylesheets
    can be tested from a git checkout:
    export XML_CATALOG_FILES="/local/git/suse-xsl/etc/catalog.xml"
    daps -d <DC> --styleroot /local/git/suse-xsl/build/stylesheet/suse2013-ns
