Ensure AWS and OpenStack CCM container names are fit for image substitution

cloud-controller-manager is currently the only name in our image substitution pattern accepts as valid. All platform manifests should follow this naming if the image is expected to be substituted.

this makes sense to me, i am starting to become a little concerned about our "api by name" convention. i think we need to make sure this naming convention is in a document somewhere in the repo, i'm worried about maintainability in the future.
