add contributing authors to book info, exports #73

Adds a 'Contributing Authors' field to Book Info. Conforms to EPUB spec: http://www.idpf.org/epub/20/spec/OPF_2.0_latest.htm#Section2.2.6

In Book Info:

@bdolor we just discovered that the duplicate "author" class here was causing issues with the CSS for running heads. Changed it here, let me know if you anticipate any issues. (FYI, @hughmcguire.)

@greatislander thank you for the head's up. Anticipated issues are minimal, and your timing is excellent. Adjustments to Luther theme may also be necessary to include styling information for the new class 'contributing-authors' on EPUB and PDF exports.
