Toolbar is not correctly positioned when parent element has position:fixed or position:absolute.

Hi. Currently, the elementsContainer isn't automatically set depending on whether a parent element of the editor is fixed, right? I reckon the toolbar is going to have to be inside that same fixed element, in order to be correctly positioned at all. Otherwise we would have to recalculate the toolbar's position in every scroll event. What do you think?

This is also true of the anchor preview bar.

Found one other toolbar positioning issue: Open demo/index.html. Resize the window to be quite narrow. Scroll all the way right. Select text to make the toolbar appear. It'll be to the left of where it should be.
