See https://github.com/dokku/dokku-couchdb/blob/master/config#L2 about where to change the targeted docker image and tag.

Also comparing the patterns used in https://github.com/frodenas/docker-couchdb#databases and https://github.com/docker-library/docs/tree/master/couchdb#specifying-the-admin-user-in-the-environment can be helpful.
