Summary

The book appointment page currently goes off the screen when viewed in a mobile browser. The form is still usable, but the user needs to scroll to read the instructions at the top and to see the submit button on some devices.

This is how to looks currently. Screenshots were taken using Edge devtools on an emulated iPhone SE screen (375px X 667px).
