I've got a nasty workaround for this, but it results in the "Select environment" (debug type) prompt appearing twice. I think it's the best we can do for now.

MS are fixing in microsoft/vscode#43133 but it could be up to a month if this isn't deemed important enough to be included in the recovery release in a weeks time.
