Add MVKConfiguration and deprecate MVKDeviceConfiguration. Add vkGetMoltenVKConfigurationMVK() and deprecate vkGetMoltenVKDeviceConfigurationMVK(). Add vkSetMoltenVKConfigurationMVK() and deprecate vkSetMoltenVKDeviceConfigurationMVK(). Add build setting overrides for all initial MVKConfiguration member values. Update to MoltenVK version 1.0.19. Update to VK_MVK_moltenvk spec version 7.
